POS0748 EASY-BILAG: A NEW TOOL FOR FASTER AND MORE ACCURATE RECORDING OF BILAG-2004 DISEASE ACTIVITY IN SLE

نویسندگان

چکیده

Background: BILAG-2004 index is an important disease activity instrument for SLE which widely used in clinical trials and treatment commissioning. It more comprehensive responsive than SLEDAI. However, may be difficult or time-consuming to complete during routine clinic visits. To derive the eventual scores from A (highly active) E (no current prior involvement) each of 9 organ domains, relies on a separate form, glossary scoring algorithm. Objectives: The Easy-BILAG project aimed develop validate simplified tool original rapidly accurately care. Methods: Data UK BILAG-Biologics Registry (BILAG-BR) were measure frequency with 97 items occurred population active SLE. These data series prototypes draft new - “Easy-BILAG”. After preliminary testing, validation study was conducted test accuracy, speed usability compared standard template. Rheumatologists specialty trainees centres around invited score timed workbook 10 exemplar case vignettes, using either reference documents. vignettes tested clinicians both frequent uncommon manifestations as well longitudinal flare remission. All workbooks contained overview detailed instructions BILAG-2004. Results: Among 2395 submissions BILAG-BR 6 most frequently scored present 20% records; arthralgia (72%), mild skin eruption (47%), moderate arthritis (38%), mucosal ulceration (34%), alopecia pleurisy / pericarditis (22%). Twenty-five less 1% assessments. therefore designed enhance visibility capture all >5% rapid single-page assessment. remaining, common items, are scored, only when necessary, second page. incorporates abridged definition immediately adjacent item. colour-blindness compatible, colour-coding system directs instantly overall A-E domain. In exercise, asked identify assign scores, A-E, domains vignettes. Twenty clinicians, range experience, have so far participated. working documents (n = 11), took 90 +/- minutes (mean SEM) complete. Clinicians 9) completed exercise significantly faster at 66 8 (p 0.05). Crucially, yielded higher percentage accuracy 95.3 0.8 % across 81.8 6.2 achieved by documentation difference apparent specifically comparing where exercises registered disease. Here, showed no decline 94.9 1.0 75.7 5.3% 0.005). survey, (9/9) testing template rated it intuitive simple navigate. Conclusion: facilitates accurate provides format amenable use practice. Following completion validation, will made available clinicians. Figure 1. The BILAG-2004 index is sensitive to change for assessment of SLE disease activity

OBJECTIVE To determine if the BILAG-2004 index is sensitive to change for assessment of SLE disease activity. METHODS This was a prospective multi-centre longitudinal study of SLE patients. At every assessment, data were collected on disease activity (BILAG-2004 index) and treatment. Analyses were performed using overall BILAG-2004 index score (as determined by the highest score achieved by a...

The BILAG-2004 systems tally—a novel way of representing the BILAG-2004 index scores longitudinally

OBJECTIVE This was an exploratory analysis to develop a new way of representing BILAG-2004 system scores longitudinally that would be clinically meaningful and easier to analyse in comparison with multiple categorical variables. METHODS Data from a multicentre longitudinal study of SLE patients (the BILAG-2004 index and therapy collected at every visit) were used. External responsiveness anal...

Numerical scoring for the BILAG-2004 index

OBJECTIVE To develop an additive numerical scoring scheme for the BILAG-2004 index. METHODS SLE patients were recruited into this multi-centre cross-sectional study. At every assessment, data were collected on disease activity and therapy. Logistic regression was used to model an increase in therapy, as an indicator of active disease, by the BILAG-2004 index score in the nine systems. As both...

BILAG-2004 index captures systemic lupus erythematosus disease activity better than SLEDAI-2000.

OBJECTIVE To assess the reliability of Systemic Lupus Erythematosus Disease Activity Index (SLEDAI)-2000 index in routine practice and its ability to capture disease activity as compared with the British Isles Lupus Assessment Group (BILAG)-2004 index. METHODS Patients with systemic lupus erythematosus from 11 centres were assessed separately by two raters in routine practice. Disease activit...
